  • The Failure of Draining: If you go to an Urgent Care or squeeze it at home, you are popping the balloon and letting the “air” out. The lump goes flat, but the balloon (the cyst wall or sac) is still there. Eventually, it will refill.

Types of Cysts We Treat

  1. Epidermoid Cysts
    • These are the most common. They often appear on the face, neck, and back but can occur anywhere on the body. They are filled with a cheesy, white substance (macerated keratin) and can sometimes smell bad if they rupture.
    • Our Approach: We use a “minimal incision” technique to remove the cyst through the smallest possible opening to minimize scarring, especially on visible areas like the face.
  2. Pilar Cysts (Scalp Cysts)

    • These are firm, smooth bumps that form on the scalp (in the hair). They often run in families.

    • Our Approach: These have a thicker wall, which actually makes them easier to “pop out” whole. I can often remove these through a tiny incision with no need to shave your hair. You walk out with your hair covering the stitches immediately.

 

The “Inflamed Cyst” Warning

If your cyst is red, hot, and painful, it has likely ruptured under the skin. Once that happens, the keratin material in the sac spills out and the immune system comes in to clean it all up and this causes inflammation. Many patients think the cyst is “infected.” It’s not an infection; it’s inflammation. 

  • Important: We usually cannot perform the full surgery while it is inflamed because the “balloon” has already popped and you don’t want to perform removal and suture on inflamed skin.

  • The Plan: We will treat the inflammation first (often with a steroid injection) to calm it down. Once it shrinks back to a quiet, painless lump (usually 4–6 weeks), we can schedule the surgery to remove the sac safely.
