You might be dealing with Perioral Dermatitis (POD; AKA periorificial dermatitis).

This frustrating condition is frequently misdiagnosed as acne or eczema. At my clinic in Layton, I see patients every day who have been struggling with this rash for months, often making it worse with the wrong treatments. What is Perioral Dermatitis?

Perioral Dermatitis literally means “inflammation around the mouth.” It presents as small, red, acne-like bumps and scaly patches.

  • The Look: It typically circles the mouth but strangely leaves a small band of clear skin right next to the lips (the vermilion border).

  • The Feel: Unlike eczema (which itches) or acne (which throbs), POD often burns, stings, or feels tight.

  • The Location: While it usually starts around the mouth, it can spread to the nose (Perinasal) and the eyes (Periocular).

 

The “Steroid Trap” (Please Read This!)

The most common cause of Perioral Dermatitis is the misuse of topical steroids.

  1. A patient gets a small rash.

  2. They apply over-the-counter hydrocortisone or a prescription steroid.

  3. The rash disappears like magic!

  4. A few days later, the rash comes back—worse than before.

  5. The patient applies more steroid.

This is a trap. Steroids suppress the inflammation temporarily but weaken the skin barrier and fuel the bacteria/mites that cause POD. To cure this, we must stop the steroids immediately.

 

Other Common Triggers

Besides steroids, POD can be triggered by:

  • Heavy Cosmetics: Thick foundations or moisturizers that occlude the skin.

  • Fluorinated Toothpaste: Sometimes switching to a natural toothpaste helps.

  • Hormonal Changes: It is most common in young and middle-aged women.

 

How We Treat It

Treating Perioral Dermatitis requires patience and a “less is more” approach. We need to repair your skin barrier, not attack it.

  1. Zero Therapy. The first step is often stopping all face creams, heavy makeup, and especially topical steroids. Your skin needs a “detox.”
  2. Oral Antibiotics (Anti-Inflammatory Dose). For moderate to severe cases, oral antibiotics (like Doxycycline or Minocycline) are the gold standard. We use them at a low dose—not to kill bacteria, but for their powerful anti-inflammatory properties. This usually clears the rash within a few weeks.
  3. Non-Steroid Topicals We use safe, non-steroidal creams to soothe the skin, such as:
    • Topical Metronidazole or Erythromycin.
    • Azelaic Acid.
    • Calcineurin Inhibitors (like Elidel or Protopic) which calm inflammation without the side effects of steroids.
